The way you are describing "upper 2 fender mount holes" is sounding alarm bells. The 1940-41 pickup only had 2 mount holes on the cowl, the lower one and an upper one that used a short brace. The fender mount holes punched in the cab side are vestigial from 1938-39.


The shape of the back of the fender looks "wrong" but I am not sure why or if it could be an optical illusion.



The attached photo shows some of the detail on the cowl without fenders.
