All the above are good suggestions. But also check that the cam (the siding part that pushes out the shoes) is actually sliding. And check to see that the rollers on the shoes are sitting correctly.

Ed - Securely tighten the nut on the bolt. Use a breaker bar. Test the pitman arm to make sure that there is absolutely no movement relative to the sector shaft.
