Default Busted Door Glass - How?

When the window was rolled up after the Fourth of July parade, this is what we saw. I happened with the window rolled all the way down. Why? The door was not slammed any harder than the usual firm shove that it takes for the door to latch.

I have purchased an OEM replacement glass for a '41 Mercury from Ebay. It is the narrow door coupe size and looks to have the same measurements as the Ford narrow door coupe. That is, about 17.5" wide by 15.5" tall.

What will I need to bed the front edge chrome strip and the bottom channel?

Any secrets to removing and replacing?
