Default Head damage, possible causes?

This is the passenger side head off of the industrial engine I have. Severe damage on the casting, but absolutely no damage to the cylinder or the piston. Just trying to figure out what happened here?
A machinist friend of mine said it could’ve been a flaw in the casting, but I don’t see how it could’ve ran for years if it was a flaw.
