Seriously, given the nature of it being for (hopefully) rare use entry and exit through the back tailgate would be a practical compromise.

That car certainly isn't going to be chasing any bad guys on a fast get-away and it would take a while to get any distance for a rescue. I bet it only goes 20 mph tops like the trucks on the thread I just posted.
