The pilot bearing is fixed in the flywheel, so, yes, the outer half turns all the time that the engine is running.
The inner half of the pilot bearing turns with the transmission input shaft. So...

Some starter Bendix drives will push the ring gear forward over time.
Remove the starter motor and inspect the ring gear on the flywheel.
Use a wire with an L bent on to the end to see if...
