Phil, glad you found the culprit. I'm a little concerned that you had a 2mm gap and a washer took up that gap. Is the washer 3mm thick? You need to be absolutely sure the bolts are applying full clamping to the plate before bottoming.

Of course if the dimensions you were quoting were generalisations and not accurate measurements you might be ok.

You might consider shortening the bolts a little if there is any sign they are still bottoming.
