Ron, I used a piece of ⅜" schedule 80 S/S tubing and tapped both ends to match the Mitchell shifter thread and inserted a short piece of all thread in one end to screw the knob onto. I used a drop of green LocTite to keep the extension tight.
I'm not sure where you can find ⅜" schedule 80 S/S tubing tho. I got it with some other scap metal.
