I've done it by removing only the hood and radiator but, am wondering if it's really necessary.

Yes, I’ve done it many times without removing the hood. Also removed and replaced the V-12 in my 39 Zephyr with hood on. The key to doing it with the hood on is keep the chain between the boom and hook short as possible and I remove the intake so I can hook as close to the engine as possible. This tool is very handy for lifting flatheads.
