Either taper could be off center. There have been water pump shaft irregularities before with some restoration parts. The fan bore taper is generally reamed but if the bore was off center then the whole thing is off center. A bent shaft or blade could also figure into the problem. Cast aluminum doesn't bend easy but it can bend on thin castings.
A dial indicator is the best way to check the shaft for off taper or runout.
