A previous bearing may have seized and spun on the spindle making the new bearing loose. Maybe someone thought the bearing wouldn't stay in place, who knows what happens sometimes.

A factory issue from Ford when I was a kid were undersized spindles/ loose bearings. Rather than replace those spindles Fords fix was to prick punch the spindle and install the bearing. Didn't seem right to me, but, it was their fix and many were done with no issues.
