Inspect the flywheel and determine the bolt pattern and pattern size. Could be 9, 9.5,10 or 11".
Most likely it has the "Long" style clutch. I agree with the above, contact Fort Wayne Clutch. They have the whole package at a very reasonable cost, including the pilot bearing, throw out bearing, alignment tool, disc and plate. Rough up the flywheel surface with a scotchbright if you find no reason to need resurfacing the flywheel. Most folks would balance the pressure plate and flywheel as well, on a budget you may be ok without that.
