I just thought of another possibility. Maybe when someone tightened the second nut, for the battery cable, they twisted the stud and first nut and caused the contact strap to rub on the side of the insulation. Center the strap and have the first nut good and tight before you install the switch, then hold the battery cable tight while you tighten the second nut, so the stud and contact strap don't move.
