David, in order from above:

— Depends on the oil filter, but a half quart is a good starting point. You can do your own experiment by filling the filter prior to installing and noting the volume. Some folks recommend this as good practice anyhow.
— As long as oil is circulating, the filter is working. A case can be made that a full flow filter might be more effective initially because all volume goes through the filter.
— That’s a reasonable statement, but again, as long as the pump is pumping oil, it’s getting filtered.
