I believe that originally Ford placed them in the upper hose down close to the water pumps and added a third hose clamp to prevent them from migrating up the hose.

I'm not sure when the change took place, but some were like those shown in the above photos, namely attached with screws and others (of both versions) were spot welded to the dash.

It should go together without applying pressure. Perhaps the plating on the bezel is now too thick on the inside perimeter for the face plate, etc. to fit without having to apply pressure.

No, they are not slotted, but they are somewhat oversize, hence the use of countersunk washers (unless it is a super-early body which had smaller holes in the pillar and no countersunk washers). ...

They are easy to misplace or half the time they are broken.

As suggested above, Michael Driskell (Third Gen Automotive/35fordtn on this site) has them.
