It sounds like you need a bracket to hold the cable jacket at the OD end.

Remember that the "clamp" is not designed to hold the tail pipe rigid, it should be a sliding fit to allow for expansion/contraction of the pipe.

IIRC #18 is only rated for 5 amps, for 11 amps #14 would be a better choice , 15 amp rating.

Probably comes from when using wood wheels, like on a Model T, there is a lug that goes between the wheel rim and the felloe of the wood wheel, and the "lug nut" tightens down against the lug....

I would suspect that the thin flat washer is to protect the shaft from getting chewed up by the lockwasher.
