Looking for Autolite "waffle-type" manifold and heater

I am looking for an Autolite heater system which includes both the "waffle-type" exhaust manifold and the bolt-on cast iron heater cover.
A matching/mated and planed intake manifold would be a bonus.

I need one for a stock Model A 4-cylinder flathead engine, not a V-8 flathead.
