It is an indent to allow more clearance for the spare tire when opening the door.

This is a Funeral Coach owned by the funeral home across the street from The Henry Ford in Dearborn. They bought this coach new and have kept it all these years.

Did that model have larger wheels/tires, so the indentation was needed?

The tires were 600 (or 650) x 20 truck tires. Even with the indent, the door still didn't swing very wide.

I spotted that on their front lawn in 2003 when we were in Dearborn for the Model-A national meet.

When the spare is sidemounted you have to have the indent. I've seen a few. Most plain late AAs have a spare under the bed or duals. The truck in Terry's illustration doesn't have one but it's not a Ford body.
