Never seen one quite like that, I know they had them in the 47 ton trucks. They did not look like that one. Below are a couple pics of the pedal out of a 47 grain truck,

I can't visualize that it could have ever started out as a 1932 4 cyl/Model B assembly (pictured).

Given the lever on the spoon end, it is more likely to have started out as a '33-'34 4-cylinder LHD assembly as shown in the top of the photo below.
