1915 Smith Form-A-Truck T?

Does anyone have an old Smith Form-A-Truck Model T?

And if so, is the value different from the average Model T?

The story goes, about 30 years ago, a buddy of my grandpa's who owed him some money called him up and said he was about to buy him something at an auction in Kansas, and if that would cover the dept. The generous man my grandpa was said "Of course". He thought he was going to get some snow-tracks for the T's, and he wanted some but what he got was the Smith Form-A-Truck, my grandpa fully restored it and loaned it to our local museum.

I can't find much information on the Smith Form-A-Truck, I know they're rare, but just how rare? Any information you can share please do so! I'd love to learn more about this fine machinery.

Not real rare, they seem to be around, with several restored in the last few years.
